When the impulse arises elsewhere than in the S-A node, e.g. in atria, A-V node or ventricles, it is referred to as an ectopic rhythm. The rhythm may be regular or irregular. If extrasystoles are discovered and are not giving rise to symptoms, the patient should be told of their presence and no treatment is required. When palpitation is complained of but there is no evidence of cardiac disease the patient should be reassured. Toxic factors, such as excessive tea-drinking or smoking, should be reduced and dyspepsia investigated. A sedative such as phenobarbitone may be given as a temporary measure. If any organic disease is associated it should be treated in the ordinary way.
